Program Details:
- **Eligibility:** The patient or dependent must have a cystic fibrosis diagnosis.
- **Referral Requirement:** A hospital professional must submit a referral describing the financial need.
- **Application Process:** Once the referral is accepted, the patient or family must complete an application.
- **Photos/Video for Fundraising:** The patient will be required to upload two photos and/or a short video to support fundraising for this critical program.
- **Review Timeline:** Please allow 3 business days for us to review and process your application. We aim to provide emergency assistance as quickly as possible.

Thank you for your interest in our Work Proudly Program (WPP)
As a social or hospital worker, your referral starts your patient's journey. Once accepted, we'll send your patient an application link to join the second year of WPP.
WPP helps individuals with CF or caregivers of children with CF in the following areas:
- One on one work with a Career Counselor
- Identify potential careers
- Cover certificate training costs for remote careers
- Provide equipment for remote work if needed
The goal of our program is to help participants gain marketable skills and secure flexible, work-from-home jobs.
Our Work Proudly Case Manager will screen all applicants to ensure they meet these qualifications:
- Confirmed Cystic Fibrosis diagnosis by a hospital professional (self or dependent)
- History of financial hardship or insufficient SSI/SSDI support
- Show an interest in learning more about the remote work industry
Participants must commit to:
- Complete all provided training if applicable
- Maintain and use equipment as intended; should they not complete the certificate training, they will be asked to return the equipment for use by another client
- Promptly answer surveys and video testimonials upon request while participating in the program
For questions, contact Melissa@clairesplacefoundation.org
Guidelines
This section must be completed by a social worker, nurse, or doctor. All information will be verified by the patient. We do this as a requirement of our 501c3 nonprofit status.
Eligibility for our Extended Hospital Stay Fund Program is as follows:
- Any child or adult diagnosed with cystic fibrosis who has experienced a 14 consecutive day hospital stay within the last calendar year. *Due to COVID, we have expanded this to include a home tune up that includes at least 14 consecutive days on IV medication or if a patient is discharged from the hospital to complete a 14 day IV medication round at home.
- At this time, we are only able to assist patients with rent or mortgage payments due to a decline in donations and an increase in need in our community.
- In order to review the request for financial assistance, a hospital professional (Doctor, Nurse or Social Worker) must complete and submit this referral form.
* Financial assistance is typically a one-time payment for approved applicants paid to a third-party vendor on the applicant’s behalf.
